Welcome to Community Health Plan

Nearly 185,000 Community Health Plan members will move to L.A. Care Health Plan. On March 29, 2011, Los Angeles County Board of Supervisors approved the recommendation to close its Knox-Keene licensed and Federally qualified full service health plan, the Community Health Plan and transition its operations into a Medical Care Service Organization (MCS) within the newly restructured DHS Ambulatory Care Network.

These changes will help the County of Los Angeles become one of the most effective and innovative health care systems in the country as it transitions to the federal and state reforms that will take effect in January 2014.

DHS’ newly formed ambulatory care organization is a key component of DHS’ health care delivery system. MCS will strengthen community partnerships, facilitate the operation of DHS medical homes and help improve better patient care through enhanced coordination and greater access to preventative health services.

The transition will also allow DHS to continue to provide services and manage the care of the Medi-Cal and In-Home Supportive Services patients. L.A. Care Health Plan will serve as the new health plan utilizing the DHS medical network for coordinated patient care.
