Healthy Families Program

  


Community Health Plan contracts with the State Managed Risk Medical Insurance Board (MRMIB) to participate as a Healthy Families Program provider. Since the program began in 1998, Community Health Plan has been designated the Community Provider Plan for Los Angeles County. This designation allows Community Health Plan to offer discounted premiums to its members. The Community Health Plan provider network consists of thousands of primary care providers and over 1,000 pharmacies in Los Angeles County. Under the Healthy Families Program, a $5.00 co-payment is required for covered services, except preventive and inpatient services.

Affordable Health Care for Children and Teens

The Healthy Families Program from the State of California provides comprehensive health, dental and vision coverage including:

  • Unlimited Physician Visits

  • Allergy Testing and Treatment

  • Specialty Healthcare

  • Well Baby Care

  • X-ray and Laboratory Services

  • Health Education

  • Prescription Drugs

  • Immunizations

  • Emergency care and Emergency Transportation

  • Hospital Inpatient Care

  • Eye Examinations and Prescription Glasses

  • Teeth Cleanings, Fillings and Other Dental Care

Who May Qualify?

  • Children up to their 19th birthday

  • Children in families with incomes at or below 250% of the Federal Income Guidelines. The Income Guidelines for your family size can be provided by one of Community Health Plan's representatives upon request. You are encouraged to apply even if it looks like your income is too high as deductions may be allowable for work, payment of child care and payment or receipt of child support or alimony.

  • Children who are U.S. citizens, nationals or eligible qualified immigrants

  • Children who are not eligible for no-cost Medi-Cal

  • Children not covered by employer-sponsored health insurance

  • Children who live in California

Costs?

  • Monthly premiums range from $4 to $15 per child to maximum of $45 for all children in family, depending on size of family and choice of health plan.

  • Most health care services are free; $5.00 co-payment for some services including prescriptions and doctor's office visits.

  • Premiums and copayments may be exempted for American Indians or Alaskan Natives.

  • Pay three months' premiums together in advance, get fourth month free

Community Health Plan, subcontracting with Health Net, L.A. Care Health Plan and community clinics, is your Community Provider Plan (CPP) in Los Angeles County (for the eighth consecutive year) offering the following features:

  • Monthly premiums ranging from $12 per child to $36 per family

  • Thousands of doctors from which to choose

  • Doctors who speak your language and understand your culture

  • Extended hours and weekend services

  • Access to wide variety of hospitals, including hospitals with special children's services

  • 1000 pharmacies; many open 24 hours

  • Urgent care centers open until 11:00 p.m., 7 days a week
