On March 27, 2001 the Los Angeles County Board of Supervisors directed the Los Angeles County Department of Health Services to develop a health benefit program under the Community Health Plan (CHP) for In-Home Supportive Services (IHSS) Workers. Effective April 1, 2002, the program was implemented. Independent IHSS providers who have worked for the last two consecutive months and have been authorized to work at least 80 hours (effective April 1, 2004) in both of the two consecutive months are eligible for this program. IHSS Workers have access to a provider network comprised of Los Angeles County Department of Health Services facilities and contract providers. A co-payment similar to that for Los Angeles County Temporary Employees Program is required.

If you have worked in Los Angeles County as an In-Home Supportive Services (IHSS) independent provider for the last two consecutive months and have been authorized to work at least 80 hours in both of the two consecutive months, you are eligible to apply for a new health care coverage program through the Community Health Plan. Simply request an Enrollment Form from the Personal Assistance Services Council (PASC) by calling 1-(877) 325-4644 to complete the form and return it to the PASC.
WHO IS COVERED?
Currently only the In-Home Supportive Services worker who meets the eligibility criteria is eligible for this coverage. However, if you have family members residing in your home, there are other health care programs offered at low cost and no cost through the Community Health Plan, such as the Medi-Cal Managed Care Program and Healthy Families Program. Community Health Plan Coverage
If PASC determines that you are eligible and we receive the completed form by the 10th of the month, your coverage as a member with the Community Health Plan will begin the first day of the following month. As a Community Health Plan member, you will be assigned a primary care provider conveniently located near your home from whom you will receive your care. You will receive an Identification Card, a Member Handbook and other informing materials to assist you in accessing health care coverage.
